Pembrolizumab + vactosertib is an investigational combination therapy consisting of two agents with complementary mechanisms of action for cancer treatment. Pembrolizumab is a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, thereby blocking its interaction with PD-L1 and PD-L2 to enhance T-cell-mediated immune responses against tumor cells. Vactosertib is a potent and selective small molecule inhibitor of transforming growth factor-beta receptor type 1 (TGFβR1), which blocks TGF-beta signaling—a pathway implicated in immunosuppression within the tumor microenvironment and resistance to immunotherapy. The rationale for combining these drugs is that dual inhibition of TGF-beta signaling and PD-1 can reverse immune evasion in tumors, particularly in microsatellite stable metastatic colorectal cancer (MSS mCRC), where single-agent immunotherapy has limited efficacy. Clinical studies have shown promising safety and efficacy signals for this combination in MSS mCRC, but development in other indications such as non-small cell lung cancer has been discontinued or shifted to alternative combinations[1][4][5][7][9].
