Pemetrexed + cisplatin is a combination chemotherapy regimen consisting of two small molecule antineoplastic agents. Pemetrexed, marketed as Alimta, is an antifolate that inhibits several enzymes involved in folate metabolism and DNA synthesis, including thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase. Cisplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. This combination is FDA-approved for the first-line treatment of malignant pleural mesothelioma (in patients not eligible for curative surgery) and non-squamous non-small cell lung cancer (NSCLC). The regimen works by disrupting DNA replication and cell division in cancer cells. It is administered intravenously in cycles every 21 days[1][2][3][4][6].
