pemetrexed + dexamethasone is a combination chemotherapy regimen administered intrathecally for the treatment of central nervous system metastases in patients with non-small cell lung cancer (NSCLC). Pemetrexed is a multi-targeted folate antimetabolite that inhibits several key enzymes in the de novo pathways of pyrimidine and purine synthesis, including thymidylate synthase (TS), dihydrofolate reductase (DHFR), and glycinamide ribonucleotide formyltransferase (GARFT). Dexamethasone is a potent synthetic glucocorticoid used in this context to reduce inflammation and edema associated with brain and leptomeningeal metastases. This specific regimen is being investigated by the Tianjin Medical University Cancer Institute and Hospital in combination with the EGFR inhibitor furmonertinib and stereotactic radiotherapy as a first-line treatment for EGFR-mutated NSCLC patients with brain parenchymal and leptomeningeal metastases.
