Pemetrexed is a folate antimetabolite chemotherapy agent that inhibits key enzymes involved in nucleotide synthesis, thereby disrupting DNA replication and repair in cancer cells. Donor-derived natural killer (NK) cell infusion is a form of adoptive cellular immunotherapy, where allogeneic NK cells are administered to the patient to enhance anti-tumor immune responses. The combination of pemetrexed with donor-derived NK cell infusion aims to leverage both direct cytotoxic effects on tumor cells and enhanced immune-mediated tumor killing. Pemetrexed has been shown to sensitize non-small-cell lung cancer (NSCLC) cells to cytotoxic immune cells by modulating the expression of immune-regulatory molecules such as PD-L1 and UL16-binding proteins (ULBP), which are ligands for the activating receptor NKG2D on NK cells[1][5]. This combination may improve anti-tumor efficacy compared to either treatment alone.
