Pemetrexed + epirubicin is a combination of two chemotherapeutic agents used in oncology. Pemetrexed is an antifolate antineoplastic agent that disrupts folate-dependent metabolic processes essential for cell replication by inhibiting enzymes such as thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase, thereby blocking DNA and purine synthesis[1][4][5]. Epirubicin is an anthracycline topoisomerase II inhibitor that exerts its cytotoxic effects by intercalating into DNA, inhibiting topoisomerase II activity, and generating free radicals leading to DNA damage[2][3][6]. Both drugs are administered intravenously and are primarily indicated for the treatment of various cancers. Pemetrexed is approved for non-squamous non-small cell lung cancer (NSCLC) and malignant pleural mesothelioma[1][4], while epirubicin is mainly used as adjuvant therapy in breast cancer with axillary node involvement but has broader antitumor activity[3][6].
