Pemetrexed + oxaliplatin is a combination chemotherapy regimen consisting of two small molecule antineoplastic agents. Pemetrexed is a multitargeted antifolate that inhibits several key enzymes involved in folate metabolism and DNA synthesis, including thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase. Oxaliplatin is a platinum-based chemotherapeutic agent that forms DNA crosslinks, inhibiting DNA replication and transcription. The combination has demonstrated synergistic antitumor effects in preclinical models and clinical activity in advanced gastric cancer, colorectal cancer, and non-small cell lung cancer (NSCLC). It is typically administered intravenously every 21 days with vitamin supplementation to reduce toxicity[1][3][4][5]. The regimen has shown efficacy as first-line therapy for advanced gastric cancer (AGC) with response rates comparable to other platinum-based regimens but without the inconvenience of prolonged infusion schedules[1]. Toxicities include neutropenia, thrombocytopenia, diarrhea, febrile neutropenia, paresthesias, mucositis, diarrhea, and hand-foot syndrome[1][5].
