This is a combination regimen consisting of three chemotherapeutic agents: - **Pemetrexed** is an antifolate antimetabolite that inhibits several folate-dependent enzymes involved in the synthesis of thymidine and purine nucleotides, thereby disrupting DNA and RNA synthesis in rapidly dividing tumor cells. - **Paclitaxel** is a microtubule inhibitor that promotes microtubule assembly from tubulin dimers and stabilizes them against depolymerization, leading to cell cycle arrest at the G2/M phase. - **Nab-paclitaxel** (nanoparticle albumin-bound paclitaxel) is a solvent-free formulation of paclitaxel bound to albumin nanoparticles. It shares the same mechanism as conventional paclitaxel but offers improved delivery to tumors via albumin-mediated transport mechanisms. The combination has been studied primarily for advanced non-small cell lung cancer (NSCLC), with preclinical data suggesting additive or synergistic cytotoxicity due to their independent mechanisms. Clinical trials have evaluated safety, tolerability, and efficacy in NSCLC patients[1][3]. Nab-paclitaxel may offer reduced toxicity compared to solvent-based formulations[1][2].
