Pemetrexed is an antifolate antineoplastic agent that inhibits multiple folate-dependent enzymes involved in the de novo biosynthesis of thymidine and purine nucleotides, thereby disrupting DNA and RNA synthesis and leading to cell death. It is primarily used for the treatment of malignant pleural mesothelioma and non-squamous non-small cell lung cancer (NSCLC), either as monotherapy or in combination with other agents[1][2][5][8]. Pelareorep is an oncolytic virus therapy based on a proprietary formulation of reovirus type 3 Dearing strain. It selectively replicates in tumor cells with activated Ras signaling pathways, causing direct tumor lysis. Additionally, it stimulates both innate (via NK cells) and adaptive immune responses against cancer cells[6]. The combination of pemetrexed plus pelareorep has been investigated in clinical trials for advanced/metastatic NSCLC[4].
