Penetropine, also known as phencynonate, is a small molecule muscarinic acetylcholine receptor antagonist developed by the Institute of Toxicology and Pharmacology at the Academy of Military Medical Sciences in China. Chemically a tropane derivative, it acts as an anticholinergic agent by competitively inhibiting the binding of acetylcholine to muscarinic receptors. The specific (R,R)-enantiomer, often formulated as a fumarate salt, has been investigated in clinical trials as an injectable treatment for the preoperative reduction of salivary and airway gland secretions. This application is intended to prevent respiratory complications such as aspiration and to maintain a clear airway during anesthesia and surgical procedures. While the racemic form of phencynonate is approved in China for the treatment of motion sickness, the development of the (R,R)-isomer focuses on optimizing the pharmacological profile for anesthetic and preoperative use.
