This combination therapy consists of a pentavalent antimonial compound, such as meglumine antimoniate, administered alongside N-acetylcysteine (NAC) as an adjuvant for the treatment of visceral leishmaniasis. Pentavalent antimonials have been the standard of care for leishmaniasis for over 40 years, working by inducing pro-inflammatory host responses and potentially disrupting the parasite's bioenergetics and redox balance through the inhibition of macromolecular synthesis. NAC is added as an adjuvant to modulate the immune response and mitigate the oxidative stress-related toxicities, including cardiotoxicity, hepatotoxicity, and nephrotoxicity, which are common adverse effects of antimonial therapy. This specific regimen was evaluated in a randomized, blinded clinical trial led by the Hospital Universitário Professor Edgard Santos to assess safety, efficacy, and immune response in patients with visceral leishmaniasis.
