Pentixather is a synthetic ligand used as the targeting moiety in radiopharmaceuticals for cancer therapy. It is most commonly conjugated with beta-emitting radionuclides such as lutetium-177 (177Lu) or yttrium-90 (90Y), resulting in agents like 177Lu-pentixather and 90Y-pentixather. These compounds are designed to target the C-X-C chemokine receptor type 4 (CXCR4), which is overexpressed in various hematological malignancies and solid tumors. Upon binding to CXCR4-expressing cells, the radiolabeled compound delivers targeted cytotoxic radiation that induces DNA damage and cell death while sparing surrounding healthy tissue. PentixaTher has been primarily developed for use in relapsed or refractory multiple myeloma but has also shown potential in other cancers with high CXCR4 expression such as lung cancer, pancreatic cancer, colorectal cancer, T-cell lymphoma, acute myeloid leukemia (AML), and central nervous system lymphoma. The drug also exhibits myeloablative effects by targeting hematopoietic stem/progenitor cells.
