Pentosan polysulfate sodium is a semi-synthetic, heparin-like macromolecular carbohydrate derivative used primarily to relieve bladder pain and discomfort associated with interstitial cystitis. It is structurally similar to glycosaminoglycans and classified as a low molecular weight heparin-like compound. The drug works by preventing irritation of the bladder wall, though its precise mechanism in interstitial cystitis is not fully understood. Pentosan polysulfate binds fibroblast growth factors (FGFs) and other heparin-binding growth factors, antagonizing their activity; it also interacts with the heparin-binding site of Fibroblast growth factor receptor 1. The drug has anticoagulant and fibrinolytic effects but is mainly indicated for genitourinary use[1][2][4][5].
