ATL-RIC is a reduced-intensity conditioning (RIC) regimen developed by the National Cancer Institute (NCI) for use prior to allogeneic hematopoietic cell transplantation (HCT). Originally designed for patients with Adult T-cell Leukemia/Lymphoma (ATL), it has also been evaluated in clinical trials for other Peripheral T-cell Lymphomas (PTCL). The regimen is a multi-drug combination consisting of the adenosine deaminase inhibitor pentostatin, the alkylating agents cyclophosphamide and busulfan, and rabbit anti-thymocyte globulin (rATG). This combination is intended to provide potent immunosuppression and cytoreduction to facilitate donor stem cell engraftment and induce a graft-versus-lymphoma effect while minimizing the treatment-related mortality associated with standard myeloablative conditioning, particularly in older or heavily pretreated patients.
