Pentoxifylline + metadoxine is a fixed-dose combination of two small molecule drugs, pentoxifylline and metadoxine. This combination has been studied primarily for the treatment of severe alcoholic hepatitis. Pentoxifylline is a methylxanthine derivative and non-selective phosphodiesterase inhibitor that improves blood flow by increasing erythrocyte flexibility, reducing blood viscosity, inhibiting platelet aggregation, and exerting anti-inflammatory effects. Metadoxine acts as an antioxidant that participates in glutathione synthesis, inhibits hepatic steatosis, modulates GABAergic neurotransmission (as a monoamine-independent GABA modulator), antagonizes serotonin 5-HT2B receptors, inhibits GABA transaminase activity (increasing synaptic GABA), and enhances acetaldehyde dehydrogenase activity to accelerate ethanol clearance. Clinical studies have shown that the combination improves short-term survival rates in patients with severe alcoholic hepatitis compared to monotherapy with either agent alone[1][8]. The combination also appears to support alcohol abstinence.
