Overview

This combination therapy consists of pentoxifylline, a xanthine derivative, and valsartan, an angiotensin II receptor blocker (ARB). Pentoxifylline is traditionally used to treat peripheral artery disease but possesses significant anti-inflammatory and anti-fibrotic properties, primarily through the inhibition of phosphodiesterases and the downregulation of pro-inflammatory cytokines such as tumor necrosis factor-alpha (TNF-alpha). Valsartan acts as a selective antagonist of the angiotensin II type 1 (AT1) receptor, which helps reduce blood pressure and provides renoprotection by decreasing intraglomerular pressure and proteinuria. The National Taiwan University Hospital has investigated this combination for its potential synergistic renoprotective effects in patients with stage 3 and 4 chronic kidney disease (CKD), aiming to slow the progression to end-stage renal disease (ESRD) and reduce cardiovascular complications.
