Peperidone hydrochloride (also known as piperphentonamine hydrochloride or Jiaobentong'an hydrochloride) is a novel small molecule cardiotonic agent developed by Guangzhou Zhongwei Biotechnology for the treatment of heart failure. It functions as a pure calcium sensitizer by increasing the sensitivity of cardiac troponin C to calcium ions, thereby enhancing myocardial contractility (positive inotropy) without increasing intracellular calcium levels or myocardial oxygen consumption. Unlike other inodilators such as levosimendan, peperidone hydrochloride does not inhibit phosphodiesterase 3 (PDE3) or increase cyclic AMP (cAMP) levels, which may reduce the risk of arrhythmias and other cAMP-mediated adverse effects. In addition to its inotropic effects, it exhibits vasodilating and cardioprotective properties, particularly against ischemia-reperfusion injury. The drug has been evaluated in Phase II clinical trials in China for hospitalized patients with acute or chronic heart failure requiring inotropic support.
