PEPITEM (Peptide Inhibitor of Trans-Endothelial Migration) is a naturally occurring 14-amino acid peptide derived from the 14-3-3 zeta protein. Discovered by researchers at the University of Birmingham, it acts as a key regulator of the PEPITEM-adiponectin pathway, which maintains immune homeostasis by controlling leukocyte trafficking. PEPITEM is produced by B-cells in response to adiponectin stimulation and binds to Cadherin-15 (CDH15) on endothelial cells. This binding triggers the synthesis and release of sphingosine-1-phosphate (S1P), which subsequently acts on S1P receptors on T-cells to inhibit their trans-endothelial migration. By preventing excessive leukocyte infiltration into tissues, PEPITEM shows therapeutic potential for chronic inflammatory and autoimmune disorders, including rheumatoid arthritis, type 1 diabetes, and multiple sclerosis.
