Perifosine is a synthetic, orally active alkylphospholipid and a novel antitumor agent that acts primarily as an allosteric inhibitor of Akt (protein kinase B), targeting the pleckstrin homology domain of Akt. By interrupting the PI3K/Akt/mTOR signaling pathway, perifosine inhibits phosphorylation and activation of Akt, leading to downstream effects such as inhibition of cell proliferation, induction of apoptosis, and modulation of membrane lipid composition. It also affects other pathways including MAPK/ERK and SAPK/JNK, contributing to its antiproliferative activity. Perifosine was developed for various cancer indications including colorectal cancer and multiple myeloma but has been discontinued in most indications following unsuccessful phase III trials. The drug was originally developed by Æterna Zentaris (now COSCIENS Biopharma) and licensed to Keryx Biopharmaceuticals[1][2][3][4][5].
