Overview

Peripheral blood hematopoietic progenitor cells (PBPCs) are hematopoietic stem cells collected from the circulating blood of a donor, typically following mobilization from the bone marrow using agents such as granulocyte colony-stimulating factor (G-CSF). In the clinical context established by the National Heart, Lung, and Blood Institute (NHLBI), these cells are utilized for allogeneic transplantation in patients with high-risk hematologic malignancies or debilitating bone marrow disorders. The therapeutic approach involves a non-myeloablative conditioning regimen—typically consisting of cyclophosphamide and fludarabine—to suppress the recipient's immune system sufficiently to allow for donor cell engraftment without the extreme toxicity of traditional high-dose radiation or chemotherapy. Once infused, the PBPCs migrate to the bone marrow to reconstitute hematopoiesis and provide a graft-versus-tumor (GVT) effect, where donor-derived immune cells identify and eliminate residual malignant cells.
