Perospirone is an atypical (second-generation) antipsychotic of the azapirone family. It acts primarily as an antagonist at serotonin 5-HT2A and dopamine D2 receptors, with additional activity as a partial agonist at 5-HT1A receptors. Perospirone also antagonizes dopamine D4, α1-adrenergic, and histamine H1 receptors. These combined actions are believed to improve both positive and negative symptoms of schizophrenia by modulating dopaminergic and serotonergic neurotransmission in various brain pathways. Developed by Dainippon Sumitomo Pharma (now Sumitomo Pharma), it was introduced in Japan for the treatment of schizophrenia and acute bipolar mania[3][4][6]. Perospirone is not approved in the US or EU but is marketed in Japan under the brand name Lullan[3][6].
