Overview

Perrhenate-188 (188ReO4-) is a therapeutic radiopharmaceutical containing the beta-emitting radioisotope rhenium-188. It is a chemical analog of iodide and serves as a substrate for the sodium/iodide symporter (NIS), a membrane protein that mediates the active transport of anions into cells. While NIS is naturally expressed in the thyroid, perrhenate-188 is being investigated for targeted internal radiation therapy (TIRT) in other malignancies, such as prostate cancer, through the use of gene therapy or engineered NIS proteins (e.g., PF-NIS) to induce tumor-specific uptake. Rhenium-188 is characterized by a high-energy beta emission (maximum energy of 2.1 MeV) and a relatively short physical half-life of approximately 16.9 hours, allowing for the delivery of high radiation dose rates to tumor tissues with a favorable safety profile compared to longer-lived isotopes.
