The **personalized mRNA neoantigen vaccine** is an investigational autologous cancer immunotherapy developed by **Shanghai Pulmonary Hospital** in collaboration with **Stemirna Therapeutics** for the treatment of **non-small cell lung cancer (NSCLC)**. This vaccine platform utilizes next-generation sequencing (NGS) to identify patient-specific somatic mutations, from which a selection of high-affinity neoantigens is predicted using bioinformatics algorithms. Synthetic mRNA encoding these neoantigens is then formulated into lipid nanoparticles (LNPs) for intramuscular administration. Upon injection, the mRNA is taken up by antigen-presenting cells (APCs), where it is translated into neoantigenic proteins. These proteins are processed and presented on MHC molecules, triggering a robust, tumor-specific T-cell response (both CD4+ and CD8+) aimed at eradicating malignant cells while sparing healthy tissue. This approach addresses the high mutational burden and heterogeneity characteristic of NSCLC.
