The personalized neoantigen polyepitope peptide vaccine is an individualized cancer immunotherapy designed to elicit a robust T-cell response against tumor-specific mutations. This specific vaccine program, often referred to as Neo-RCC in the context of renal cell carcinoma, is developed by Mingzhibenyuan Medical Technology and investigated by Jian Lin at Peking University First Hospital. The manufacturing process involves performing whole exome sequencing (WES) and transcriptome sequencing (RNA-seq) on a patient's tumor tissue to identify unique mutations (neoantigens). From these, 10 to 30 synthetic long peptides (SLPs) are selected based on their predicted binding affinity to the patient's HLA molecules. The vaccine is administered subcutaneously in combination with the adjuvant Poly-ICLC (a TLR3 agonist) to enhance immunogenicity. It is currently being evaluated in Phase I clinical trials for patients with advanced, recurrent, or refractory renal cell carcinoma, frequently in combination with the checkpoint inhibitor pembrolizumab to overcome tumor-induced immunosuppression.
