Personalized target-selected CAR-T cells are a form of autologous cell therapy in which a patient's own T lymphocytes are genetically engineered to express chimeric antigen receptors (CARs) that specifically recognize tumor-associated antigens selected based on the individual’s cancer profile. This approach aims to enhance specificity, efficacy, and safety by tailoring the antigen targets for each patient, thereby reducing off-tumor toxicity and improving anti-tumor responses. The process involves isolating T cells from the patient, modifying them ex vivo to express CARs directed against one or more tumor-specific antigens identified through molecular profiling of the patient's cancer, expanding these modified T cells, and then infusing them back into the patient. Personalized target selection is particularly relevant for solid tumors with heterogeneous antigen expression but can also be applied in hematological malignancies[1][3][4]. The therapy is currently being evaluated in early-phase clinical trials.
