Overview
PETHEMA LAL-2019 is a pediatric-inspired, risk-adapted chemotherapy regimen developed by the PETHEMA Foundation for adult patients with Philadelphia chromosome-negative (Ph-negative) acute lymphoblastic leukemia (ALL). The protocol, currently under investigation in a multicenter Phase 3 trial (NCT04179929), utilizes a multi-phase approach consisting of induction, early and delayed consolidation, reinduction, and maintenance. Treatment intensity is tailored based on measurable residual disease (MRD) levels and baseline genetic risk factors. Patients achieving early MRD clearance with standard genetic risk continue with chemotherapy blocks, while high-risk patients or those with persistent MRD are allocated to allogeneic hematopoietic stem cell transplantation (alloHSCT). The regimen incorporates a combination of cytotoxic agents including vincristine, prednisone, pegaspargase, daunorubicin, methotrexate, cytarabine, and mercaptopurine.
