Petrolatum is a purified mixture of semi-solid saturated hydrocarbons obtained from petroleum. It functions primarily as an occlusive emollient by forming a hydrophobic barrier on the skin or ocular surface, which significantly reduces transepidermal water loss (TEWL) and promotes stratum corneum hydration. In clinical settings, petrolatum-based ointments (such as Aquaphor or Vaseline) are widely used as "best supportive care" or control agents for various dermatological conditions, including radiation-induced dermatitis, atopic dermatitis, and psoriasis. While traditionally considered chemically inert, recent research suggests that petrolatum may actively induce barrier repair and antimicrobial peptide expression in the skin. It is also utilized in ophthalmic ointments to manage dry eye by stabilizing the lipid layer of the tear film and as a vehicle for slow-release drug delivery to the ocular surface.
