PF-07257876 is a novel, Fc-active, IgG1 bispecific monoclonal antibody developed by Pfizer. It targets both CD47 and programmed death-ligand 1 (PD-L1) on tumor cells. By simultaneously binding to these two immune checkpoint proteins, the drug is designed to block the "don't eat me" signal mediated by CD47 and inhibit PD-L1-mediated T-cell suppression, thereby enhancing both innate and adaptive antitumor immune responses. The dual targeting aims to reverse resistance to immuno-oncology therapies (IO-R), decrease anemia risk associated with anti-CD47 therapies, and eliminate the need for priming doses. In clinical studies, it demonstrated high target engagement in peripheral blood for both PD-L1 (>95%) and CD47 (>70%), with evidence of cytokine modulation but only modest antitumor activity before its discontinuation[2][4][7].
