PHA-794428 is a pegylated version of somatropin (recombinant human growth hormone), designed to provide a long-acting alternative to daily growth hormone injections. The drug consists of somatropin covalently linked to a 40 kDa polyethylene glycol (PEG) moiety, which significantly increases its half-life and area under the curve compared to unmodified somatropin. This modification reduces renal clearance and receptor-mediated clearance due to increased molecular size and decreased affinity for the growth hormone receptor. PHA-794428 acts as an agonist at the growth hormone receptor (GHR), mimicking endogenous human growth hormone activity. It was developed primarily for indications such as pituitary dwarfism and growth hormone deficiency in both children and adults. Clinical development reached phase 2 but was discontinued before approval[1][2][3][6].
