Phenobarbital is a long-acting barbiturate used primarily as an anticonvulsant and sedative. It was first introduced in 1912 and remains one of the oldest antiepileptic drugs still in use. Phenobarbital acts by enhancing the activity of gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter in the central nervous system, leading to increased neuronal inhibition and reduced excitability. It is recommended by the World Health Organization for treating certain types of epilepsy—especially in developing countries—and is commonly used for neonatal seizures in developed countries. Other uses include management of insomnia, anxiety disorders, withdrawal from other barbiturates or benzodiazepines, preoperative sedation, and occasionally for status epilepticus when other treatments fail. Phenobarbital can be habit-forming with potential for abuse and withdrawal symptoms upon discontinuation[1][2][5][8].
