Overview

Phenprocoumon + ticagrelor + aspirin is a triple antithrombotic therapy (TAT) regimen being investigated for the management of patients with co-existing atrial fibrillation (AF) and acute coronary syndrome (ACS). This combination therapy aims to provide comprehensive protection against both thromboembolic stroke (via the vitamin K antagonist phenprocoumon) and coronary stent thrombosis or recurrent myocardial infarction (via the dual antiplatelet therapy of ticagrelor and aspirin). Phenprocoumon acts by inhibiting the Vitamin K epoxide reductase complex, thereby reducing the synthesis of active clotting factors II, VII, IX, and X. Ticagrelor is a direct-acting, reversible P2Y12 receptor antagonist that prevents ADP-mediated platelet activation, while aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) to block the production of thromboxane A2. The Medical University of Vienna has conducted clinical research to evaluate the pharmacodynamic profile and safety of this specific triple combination, particularly its impact on coagulation markers in healthy subjects.
