Overview

Phentolamine + lidocaine is a combination pharmacological intervention developed by the 988th Hospital of Joint Logistic Support Force of PLA for use during transradial coronary procedures. The combination is designed to mitigate radial artery spasm (RAS), a common complication of radial artery catheterization that can lead to procedural failure and patient discomfort. Phentolamine acts as a non-selective alpha-adrenergic antagonist, blocking alpha-1 and alpha-2 receptors on vascular smooth muscle to induce vasodilation and counteract catecholamine-induced vasoconstriction. Lidocaine, a local anesthetic, blocks voltage-gated sodium channels to provide regional anesthesia at the puncture site and may further stabilize the arterial wall against mechanical stimulation. This spasmolytic cocktail is typically administered via local infiltration or intra-arterial injection to ensure procedural success and improve patient outcomes in interventional cardiology.
