Overview

Phenylephrine is a selective alpha-1 adrenergic receptor agonist used primarily as a decongestant for the relief of nasal discomfort caused by colds and allergies. It is also indicated for sinus congestion and pressure. In addition to its use as an oral or nasal decongestant in over-the-counter products (such as Sudafed PE), phenylephrine is administered intravenously to manage hypotension during anesthesia and topically or ophthalmically to induce mydriasis (pupil dilation) or relieve eye redness. Its mechanism of action involves direct stimulation of alpha-1 adrenergic receptors on vascular smooth muscle leading to vasoconstriction; this reduces swelling in the nasal passages and increases blood pressure when given systemically[1][3][4][5]. Phenylephrine exhibits minimal beta-adrenergic activity[4]. It does not treat the underlying cause of symptoms but provides symptomatic relief.
