Phosphate is an essential inorganic electrolyte and mineral that plays a critical role in bone mineralization, cellular energy metabolism (as a component of ATP), and acid-base balance. As a therapeutic agent, it is administered as various salts (such as sodium or potassium phosphate) to treat or prevent hypophosphatemia, a condition characterized by low serum phosphorus levels. Hypophosphatemia can result from renal wasting (e.g., in tumor-induced osteomalacia or genetic rickets), malabsorption, or as a complication of continuous renal replacement therapy (CRRT). Conversely, in the context of chronic kidney disease (CKD), phosphate levels are strictly managed because hyperphosphatemia (elevated phosphate) is a major risk factor for vascular calcification and cardiovascular disease. Therapeutic phosphate is available in oral and intravenous formulations and is also used in specialized hemofiltration replacement solutions.
