Phospho-aspirin (MDC-22) is a **novel modified aspirin derivative** designed to improve efficacy and reduce gastrointestinal toxicity compared to standard aspirin. It consists of aspirin chemically modified by the addition of a phospho group, intended to enhance its anticancer and anti-inflammatory properties while minimizing adverse effects such as gastric ulceration. Mechanistically, phospho-aspirin retains the classic aspirin action of **irreversible inhibition of cyclooxygenase enzymes (COX-1 and COX-2)** and suppression of **prostaglandin and thromboxane synthesis**, but it also demonstrates increased ability to induce apoptosis and inhibit cell proliferation in cancer models. Early-stage studies suggest its primary indication may be for **cancer chemoprevention and therapy** (especially colon and pancreatic cancers), although inflammatory and cardiovascular applications may also be possible. It is not yet approved for clinical use, and is under preclinical and early clinical development.
