Photochemically treated donor T-cells are an experimental cellular therapy developed by the M.D. Anderson Cancer Center for use in patients undergoing haploidentical (mismatched) hematopoietic stem cell transplantation (HSCT) for hematologic malignancies. The process involves treating donor T-cells ex vivo with a psoralen derivative, such as 8-methoxypsoralen, followed by exposure to long-wavelength ultraviolet A (UVA) light. This photochemical treatment is designed to selectively inactivate alloreactive T-cells that cause Graft-versus-Host Disease (GvHD) by inducing covalent DNA cross-linking, which prevents the proliferation of cells activated by host antigens. The therapeutic goal is to allow for the safe infusion of donor T-cells to promote rapid immune reconstitution and graft-versus-tumor effects while minimizing the risk of severe GvHD in the setting of HLA-mismatched transplants.
