Overview

Phylloquinone (vitamin K1) is a naturally occurring fat-soluble vitamin being investigated by Assiut University in a Phase 4 clinical trial (NCT05060809) for its potential to treat vascular calcification in patients undergoing hemodialysis. It serves as an essential cofactor for the enzyme gamma-glutamylcarboxylase (GGCX), which facilitates the post-translational carboxylation of vitamin K-dependent proteins. A key target in this process is matrix Gla-protein (MGP), a potent inhibitor of soft tissue and vascular calcification. In patients with chronic kidney disease, vitamin K deficiency often leads to high levels of inactive dephosphorylated-uncarboxylated MGP (dp-ucMGP). Supplementation with phylloquinone aims to activate MGP, thereby inhibiting the progression of aortic and heart valve calcification.
