Piclamilast

Piclamilast is a selective phosphodiesterase-4 (PDE4) inhibitor with potent anti-inflammatory effects. It acts by selectively inhibiting all four PDE4 isoforms (PDE4A-D), which are key enzymes in inflammatory and immunomodulatory cells such as mast cells, neutrophils, basophils, eosinophils, T lymphocytes, macrophages, and structural cells like sensory nerves and epithelial cells. By blocking PDE4 activity, piclamilast prevents the hydrolysis of cyclic adenosine monophosphate (cAMP) to inactive AMP, resulting in increased intracellular cAMP levels that suppress immune and inflammatory cell activity. Piclamilast has been investigated for the treatment of chronic obstructive pulmonary disease (COPD), bronchopulmonary dysplasia, asthma, dermatitis, and rheumatoid arthritis. Vomiting/emesis is the most commonly cited side effect[1][2][3][5].
