Picoplatin + docetaxel is an investigational **combination chemotherapy regimen** being studied as a first-line treatment for **metastatic castration-resistant prostate cancer** (CRPC), also referred to as hormone-refractory prostate cancer (HRPC). Picoplatin is a novel, next-generation platinum-based chemotherapy agent designed to overcome resistance associated with older platinum drugs and has shown a more favorable safety profile, particularly reduced nephrotoxicity and neurotoxicity compared to conventional platinum agents. Docetaxel is a taxane-class microtubule inhibitor widely used in the treatment of various solid tumors. The combination works by inhibiting DNA replication (picoplatin, via DNA crosslinking) and disrupting microtubule function (docetaxel), which together result in cancer cell death. Clinical trials indicate that the combination, often administered with daily prednisone, can yield high rates of PSA response, improved progression-free survival, and acceptable hematologic toxicity, with manageable adverse events and no increase in neurotoxicity. Current data support the further development of this regimen for advanced prostate cancer patients who have few effective treatment options[1][3][5].
