Overview

PIGLETS is a multi-agent combination chemotherapy regimen developed by The University of Hong Kong for the treatment of peripheral T-cell lymphomas (PTCLs) and extranodal NK/T-cell lymphomas. It was designed as a more tolerable or effective alternative to the SMILE regimen. The acronym PIGLETS stands for its constituent drugs: cisplatin (a platinum-based DNA cross-linking agent), ifosfamide (an alkylating agent), gemcitabine (a pyrimidine antimetabolite), L-asparaginase (an enzyme that depletes systemic asparagine), etoposide (a topoisomerase II inhibitor), and dexamethasone (a corticosteroid). This regimen is utilized in both newly diagnosed and relapsed or refractory settings, targeting malignant T and NK cells through multiple synergistic pathways including DNA damage, inhibition of DNA synthesis, and metabolic deprivation.
