Overview

PIGR-AAV is an adeno-associated virus (AAV)-based gene therapy developed by AstraZeneca to restore the expression and function of the polymeric immunoglobulin receptor (PIGR) in the airway epithelium. PIGR is a critical protein responsible for the transcytosis of secretory IgA (SIgA) across epithelial cells into the airway lumen, where it provides broad mucosal protection against respiratory pathogens. In patients with Chronic Obstructive Pulmonary Disease (COPD), PIGR expression is often significantly reduced, which correlates with increased exacerbation frequency and impaired mucosal immunity. By delivering a functional PIGR gene via an AAV vector, this therapy aims to boost mucosal defense and protect against viral infections, such as influenza. Preclinical studies in PIGR-deficient mice and human nasal epithelial cells have demonstrated that PIGR-AAV induces long-lived PIGR expression and functional IgA transport, leading to a significant reduction in viral load upon exposure to influenza virus.
