Pimavanserin is an atypical antipsychotic indicated for the treatment of hallucinations and delusions associated with Parkinson's disease psychosis. Unlike most other antipsychotics, it does not block dopamine receptors but acts as a selective inverse agonist and antagonist at serotonin 5-HT2A receptor, with lower affinity for serotonin 5-HT2C receptor. This unique mechanism allows it to treat psychotic symptoms in Parkinson’s disease without worsening motor function or causing extrapyramidal side effects. Pimavanserin was first approved by the FDA in 2016 and is available as oral tablets and capsules[1][4][5][6].
