PIPE-307 is a highly selective, brain-penetrant small molecule antagonist of the muscarinic acetylcholine receptor M1 (also known as CHRM1 or M1R). It is being developed primarily for the treatment of relapsing-remitting multiple sclerosis (RRMS), with additional investigation in depression. The drug works by blocking the M1 receptor, which is highly expressed on oligodendrocyte precursor cells (OPCs) in the central nervous system. Inhibition of this receptor removes an inhibitory signal on OPC maturation, thereby promoting their development into myelin-producing oligodendrocytes and facilitating remyelination of nerve fibers. Preclinical studies have shown that PIPE-307 increases myelin repair and improves functional recovery in mouse models of MS, and it has also demonstrated remyelination activity in human brain tissue samples. The drug is currently undergoing Phase 2 clinical trials for RRMS[1][3][4][5][6][7].
