Overview

Pipecuronium is a long-acting, non-depolarizing neuromuscular blocking agent of the aminosteroidal class. It is used as an adjunct to general anesthesia to provide skeletal muscle relaxation during surgery and to facilitate endotracheal intubation. Pipecuronium acts as a competitive antagonist at nicotinic acetylcholine receptors at the neuromuscular junction, preventing acetylcholine from triggering muscle contraction and resulting in reversible paralysis of skeletal muscles. Its mechanism involves both antagonism of acetylcholine at postsynaptic nicotinic receptors and inhibition of acetylcholine release at presynaptic sites. Pipecuronium was first approved in 1990 under the brand name Arduan and has been discontinued in some countries due to concerns about residual postoperative neuromuscular block but remains available in others such as China, Russia, Brazil, and Hungary[1][2][4][5][6].
