Overview
Piperacillin + tazobactam + levofloxacin is an antibiotic combination therapy being evaluated for the empirical treatment of severe community-acquired pneumonia (sCAP). This regimen combines a broad-spectrum ureidopenicillin (piperacillin) with a beta-lactamase inhibitor (tazobactam) and a respiratory fluoroquinolone (levofloxacin). Piperacillin exerts bactericidal activity by inhibiting penicillin-binding proteins (PBPs), thereby disrupting cell wall synthesis, while tazobactam extends its spectrum against beta-lactamase-producing organisms. Levofloxacin provides additional coverage, particularly against atypical pathogens and Pseudomonas aeruginosa, by inhibiting bacterial DNA gyrase and topoisomerase IV. This specific combination is the subject of the ACTSCAP trial led by Choseok Yoon in South Korea to determine if combination therapy is superior to piperacillin/tazobactam monotherapy in patients with sCAP.
