The term standard antibiotics-Herlev Hospital-perforated appendicitis refers to the control arm regimen used in a pivotal quasi-randomized clinical trial (NCT03435900) conducted at Herlev and Gentofte Hospital, University of Copenhagen. This regimen consists of the intravenous administration of piperacillin/tazobactam and metronidazole for the treatment of perforated (complicated) appendicitis. Piperacillin is a broad-spectrum ureidopenicillin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Tazobactam is a beta-lactamase inhibitor that protects piperacillin from degradation by various bacterial enzymes. Metronidazole is a nitroimidazole antimicrobial that, upon intracellular reduction in anaerobic bacteria, forms reactive intermediates that bind to and disrupt DNA, leading to strand breakage and cell death. This combination provides comprehensive coverage against the gram-positive, gram-negative, and anaerobic pathogens typically associated with intra-abdominal infections.
