Pirtobrutinib + rituximab is a combination therapy being investigated for the treatment of B-cell malignancies, specifically indolent clinical forms of mantle cell lymphoma (MCL). Pirtobrutinib is a highly selective, non-covalent (reversible) Bruton's tyrosine kinase (BTK) inhibitor that remains effective in the presence of C481 mutations, which typically confer resistance to covalent BTK inhibitors. Rituximab is a chimeric monoclonal antibody that targets the CD20 antigen on the surface of B-lymphocytes, inducing cell death through antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). The Grupo Español de Linfomas y Transplante Autólogo de Médula Ósea (GELTAMO) is sponsoring a Phase II trial (GELTAMO-PIRTO-1) to evaluate this combination in treatment-naïve patients with indolent MCL.
