pirtobrutinib + sintilimab + rituximab + methotrexate

This combination therapy, evaluated in the PRIME-PCNSL trial led by Tongji Hospital, is an investigational regimen for treatment-naive patients with primary central nervous system lymphoma (PCNSL). It integrates four distinct therapeutic mechanisms: pirtobrutinib, a highly selective, non-covalent (reversible) Bruton's tyrosine kinase (BTK) inhibitor; sintilimab, a monoclonal antibody targeting programmed cell death protein 1 (PD-1) to enhance anti-tumor immune response; rituximab, a monoclonal antibody targeting CD20 on B-cells; and methotrexate, a high-dose antimetabolite chemotherapy that crosses the blood-brain barrier. The regimen aims to improve complete remission rates and survival outcomes compared to standard-of-care treatments like the MATRix or RMT protocols.
