Pituitary adenylate cyclase-activating polypeptide-38 (PACAP-38) is a 38-amino acid endogenous neuropeptide that acts as a potent agonist at the PAC1, VPAC1, and VPAC2 receptors. In clinical research conducted by the Danish Headache Center, PACAP-38 is utilized as a pharmacological provocation tool to experimentally induce migraine attacks, including migraine with aura, in susceptible individuals. Administered via intravenous infusion, PACAP-38 triggers a cascade of physiological responses, including vasodilation and activation of the trigeminovascular system, which mimics naturally occurring migraine pathophysiology. This experimental model is crucial for investigating migraine mechanisms and evaluating the efficacy of novel therapeutic candidates targeting the PACAP pathway, such as anti-PACAP antibodies.
