Overview
Pixantrone + cyclophosphamide + vincristine + rituximab + prednisone is a multi-agent chemotherapy regimen used primarily in the treatment of relapsed or refractory aggressive non-Hodgkin lymphoma. This combination, sometimes referred to as R-CPOP, substitutes pixantrone for doxorubicin in the standard R-CHOP regimen. Pixantrone is an anthracenedione with reduced cardiotoxicity compared to doxorubicin and acts as a DNA intercalator and topoisomerase II inhibitor. Cyclophosphamide is an alkylating agent that crosslinks DNA, vincristine inhibits microtubule formation, rituximab is a monoclonal antibody targeting CD20 on B cells leading to cell lysis, and prednisone is a corticosteroid with lympholytic effects. The combination aims to maximize anti-tumor activity through complementary mechanisms while reducing cardiac toxicity associated with anthracyclines[1][5].
