Overview

Plasma-lyte is a sterile, nonpyrogenic isotonic crystalloid solution for intravenous infusion that contains a balanced mixture of electrolytes designed to closely mimic the composition of human plasma. The typical formulation includes sodium chloride, sodium gluconate, sodium acetate trihydrate, potassium chloride, and magnesium chloride hexahydrate in water for injection. It is used primarily as a source of water and electrolytes for fluid replacement in clinical situations such as burns, trauma, surgery (intraoperative fluid replacement), hemorrhagic shock requiring rapid blood transfusion compatibility with blood products, and mild to moderate metabolic acidosis including cases where lactate metabolism is impaired[1][2][3][4][5]. Acetate and gluconate serve as bicarbonate precursors providing an alkalinizing effect by being metabolized to carbon dioxide and water[1][3]. The main pharmacological action is expansion of the extracellular compartment (interstitial and intravascular fluids) with minimal risk of acid-base disturbance due to its physiologic pH (~7.4) and osmolarity (~295 mOsm/L)[2][3].
